With support coordination, we can help you navigate through your NDIS plan and assist you in finding the best solution. As independent service providers, Attentive Care allows you the complete freedom to choose your service providers. We will support you in your search for a suitable provider and then work on reaching an agreement with the providers you choose to engage with.
We understand that choosing a service provider can be challenging and our goal is to ensure that all your service providers work collaboratively and efficiently towards your support needs. Our involvement also aims to give you a solid understanding of your plan, allowing you to be involved and maximise your benefits. Our support coordination service is available to all participants aged between 7 to 65 years.
How can we support you?
- • Assistance with developing personalised support plans
- • Educate and empower you with information about NDIS, your funding benefits and your plan
- • Search for suitable service providers such as accommodation services, employment services, therapy options, recreational activities, community services, etc.
- • Help you navigate through service agreements and funding advice
- • Prepare for your NDIS reviews as needed
- • Work with you towards your personal goals and support your participation in the community
- • Support you in improving your independence, as desired by you
